EMIT法检测环孢素血药浓度的动态飘逸校正系数研究(1)

    摘 要 目的:探讨酶放大免疫测定技术(EMIT)检测环孢素血药浓度的动态飘逸校正系数。方法:针对EMIT法检测环孢素药物浓度的动态飘逸现象,以质量浓度为100 ng/mL的环孢素定标液为待测样品,选择其第58、101次检测结果来进行动态飘逸相关系数的推拟;分别以上述系数对第1、20、45、77、106、115次检测结果进行校正,考察校正质量浓度与真实质量浓度的差异。结果:推拟得动态飘逸系数k=(1+0.001 235×n0.419 5)n,动态飘逸校正系数k′ =1/[(1+0.001 235×n0.419 5)n]。各次检测结果经校正后,其校正质量浓度与真实质量浓度的相对误差在±4%之内。结论:推拟所得的动态飘逸校正系数可用于环孢素质控样品浓度的动态校正,有助于环孢素血药浓度的准确检测。

    ABSTRACT OBJECTIVE: To explore the dynamic upwarp state correction coefficient of cyclosporine blood concentration determined by EMIT. METHODS: The dynamic upwarp state of cyclosporine concentration was determined by EMIT. The cyclosporine calibration reagents with concentration of 100 ng/mL was used as the sample to be measured, the 58th and 101st test results were selected to simulate the dynamic upwarp state related coefficients, and the 1st, 20th, 45th, 77th, 106th, 115th test results were corrected by above coefficients. The difference of correction quality concentration with real quality concentration was investigated. RESULTS: The dynamic upwarp state coefficient [k=(1+0.001 235×n0.419 5)n] and dynamic upwarp state correction coefficient {k′ =1/[(1+0.001 235×n0.419 5)n} were obtained. After determination results were corrected, relative error of corrected quality concentration with real quality concentration was within ±4%. CONCLUSIONS: The obtained dynamic upwarp state correction coefficient can be used for dynamic calibration of cyclosporine quality control concentration, which contribute to correct determination of blood concentration of cyclosporine.

    环孢素是临床常用的免疫抑制剂之一,主用于肝、肾、骨髓等器官/组织移植以及再生障碍性贫血、肾病综合征、干燥综合征等异常免疫性疾病的治疗[1-6]。由于环孢素的生物利用度及肝消除等个体差异大、治疗窗窄,容易导致治疗无效或中毒[7-8],故需通过治疗药物监测(Therapeutic drug monitoring,TDM)来指导其临床个体化合理用药。

    酶放大免疫测定技术(Enzyme multiplied immunoassay technique, EMIT)是目前临床TDM的主要手段之一,具有自动化程度高,可多品种、多样本连续检测,检测时间短,样品处理简单,规律性强等特点,且当试剂瓶中的试剂较多时,相邻质控样品检测的重现性较好[9]。但该法所用试剂昂贵,且当试剂瓶中的试剂较少时,检测结果会出现偏差,重现性较差[10-12]。上述偏差的规律性很强,呈现动态飘逸现象(即EMIT法檢测环孢素全血样品时,随着检测次数的增加、试剂瓶中试剂的挥发,其质控样品的实测质量浓度会持续向上飘逸,且试剂越少,飘逸值越大,特别是在批量检测次数较少的试验中尤为突出)[12]。过去常用多次定标或质控来纠正偏差,但必然会造成大量的试剂浪费,增加检测成本。因此,为获取准确的环孢素血药浓度检测数据,本研究探讨了EMIT法检测人全血环孢素药物浓度的动态飘逸相关系数,以动态校正环孢素实测质量浓度,现报道如下。

    1 材料

    1.1 仪器
