深度烧伤患者弹力绷带压力治疗对增生性瘢痕恢复情况的影响(1)

     [摘要]目的:观察弹力绷带压力治疗对深度烧伤患者增生性瘢痕恢复情况的影响。方法:选取笔者医院2014年1月-2016年12月收治的86例深度烧伤后增生性瘢痕患者,按随机数表法分为观察组与对照组,各43例。对照组使用硅酮凝胶喷雾剂,观察组在对照组基础上采取弹力绷带压力治疗,均连续治疗12个月。采用温哥华瘢痕量表(Vancouver scar scale, VSS)评估两组治疗前、治疗12个月后的瘢痕情况,采用超声检测治疗前、治疗3个月、治疗6个月、治疗12个月的增生性瘢痕厚度,比较两组治疗前、治疗12个月后的血清组胺、羟脯氨酸、5-羟色胺水平。结果:治疗后,两组VSS各维度评分与总评分均较治疗前显著降低,观察组明显低于对照组,差异均有统计学意义(P<0.05)。治疗后3个月,两组增生性瘢痕厚度继续增厚,至治疗后6个月开始变薄。观察组治疗后3个月、6个月、12个月的增生性瘢痕厚度均低于对照组,差异有统计学意义(P<0.05)。两组的血清组胺、5-羟色胺水平与治疗前比较,差异无统计学意义(P>0.05),且组间比较差异均无统计学意义(P>0.05);两组血清羟脯氨酸较治疗前降低,观察组血清羟脯氨酸水平低于对照组,差异有统计学意义(P<0.05)。结论:弹力绷带压力治疗深度烧伤后增生性瘢痕可进一步抑制胶原纤维形成,改善增生性瘢痕的色泽、柔软度及厚度,为瘢痕恢复创造有利条件,疗效满意。

    Abstract: Objective To observe the effect of elastic bandage pressure treatment on the recovery of hypertrophic scars in deep burn patients. Methods A total of 86 patients with hypertrophic scar after deep burn admitted to our hospital from January 2014 to December 2016 were enrolled and divided into the observation group and the control group according to the random number table method, with 43 cases in each group. The control group was treated with silicone gel spray, and the observation group was treated with elastic bandage pressure on the basis of the control group. Both groups were treated for 12 months. The scar condition in two groups before and 12 months after treatment were evaluated by the Vancouver scar scale(VSS). The thickness of hypertrophic scar before treatment, 3 months, 6 months and 12 months were detected by ultrasonography. Serum histamine, hydroxyproline and 5-serotonin levels were compared between the two groups before treatment and 12 months after treatment. Results After treatment, VSS scores and total scores of the two groups were significantly lower than before treatment, the observation group was significantly lower than the control group, the differences were statistically significant(P<0.05). The thickness of the two groups of hypertrophic scars continued to increase at 3 months after treatment, and became thinner at 6 months after treatment. The thickness of hypertrophic scar at 3 months, 6 months and 12 months after treatment in the observation group was lower than that in the control group(P<0.05). There was no significant difference in serum histamine and serotonin levels between the two groups before and after treatment, and there was no significant difference between the two groups(P>0.05). The serum hydroxyproline level of the two groups was lower than that before treatment, and the serum hydroxyproline level of the observation group was lower than that of the control group(P<0.05). Conclusion The elastic bandage pressure treatment of hypertrophic scars after deep burn can further inhibit the formation of collagen fibers, improve the color, softness and thickness of hypertrophic scars, and create favorable conditions for the recovery of scars., 百拇医药(谢彬)
